Rule 11: Teeing Ground

While playing in a stroke play competition, a fellow competitor in my group teed up his ball and played it from about an inch in front of the tee markers. No one brought this to his attention until we reached our tee shots in the fairway. We thought it was a two-stroke penalty and play the ball as it lies. We kept playing and the competitor added two strokes to his score on the hole. When we discussed the situation in the scoring tent with the rules official, the competitor was told he was disqualified. Why?
